Wound clinic will review charts for scheduling of wound clinic appointments on Thursdays (changed from Wed mornings)
To make a referral to wound clinic, fill out an MR-191 and chart check the chart to Thursday morning Wound Clinic.
When a referral is made, ask the nurse to add the following to the notes section in EPIC:
1. Wound location
2. Size and number of wounds
3. Etiology of wounds, if known
4. Whether the patient uses a wheelchair, guerney, needs assistance with transfers, or ambulates
5. If the patient has dementia or is non-verbal
#4 and 5 are extremely helpful in assuring we have adequate staff and rooms to accomodate patients.
Options if the patient needs to be seen immediately or sooner than the next wound clinic appt include: surgery clinic, FPC clinic, short notice clinic, cast tech for unna boots, or calling Jennifer Zollars at x4291 (voice mail checked every Friday).
